DSL redesigns Approved Absence Forms


To ease the monitoring of student absences, the Department of Student Life (DSL) recently standardized the Approved Absence Forms which allows students to use it for a whole term.

To ease the monitoring of student absences, the Department of Student Life (DSL) recently standardized the Approved Absence Forms which allows students to use it for a whole term. These forms are now printed on white paper, and were released during the last week of September.

The updated Approved Absence Forms relatively requires the same information such as the name of the student, ID number, name of activity (where one will be sent to), duration, name of organizer, office, details of number of approved and unapproved absences, and subjects that will be affected by the absence.

 

As mentioned by DSL Dean Norie Ador Dionisio, the intention is to have only one Approved Absence Form per student and which is valid for one term. “Since, there is only one form, we are hoping that students will be more responsible in handling the form as not to lose them. It also minimizes printing costs, unlike before,” she added.

 

In conjunction to her statement, students are held responsible for completing and managing the Approved Absence Form. As stated in Section 3.6.7.3 of the Student Handbook 2016-2019, the accomplished form should be submitted before the date of the intended absence. In cases where advance notice is not possible, the activity coordinator shall explain the situation through writing to the teachers concerned, which is then noted by the DSL dean or school dean.

Absences incurred by students who are required to attend formation programs or officially representing the College as a delegate to an athletic meet/athletic-related activities, training/seminar, cultural performance, competition and similar events are considered an approved absence, per Section 3.6.7 of the Student Handbook.
